The key here really is to prevent this patient from getting intubated. We know that once a patient's intubated in the cardiac IC, their mortality goes through the roof. The dose response curve for diuretics is sinusoidal. So if you gave 40 and then respond, you would want to give 80. If they're not making 200 plus CCs a urine, you want to redose.
