An internal state department accountability review is completed and classified secret, but it's made public three years later. The report said because of an approach of, quote, excessive secrecy that contributed to a delayed response. What the heck is going on with these agencies? To figure out where the process went so wrong, we made a couple of calls. First was to Frank Figluzzi. We realized there was more than enough there in these studies to warrant a much deeper dive on what had been done.
