How Do They Find Evidence of Bias?

CNN's John Sutter asks: How do they identify it in the first place? What are the markers? How do they score things? Is it an on an annual basis or are you tagged forever with this low rating based on past sins? Or does it go from month to month? It seems like it's just a kind of mark of cane that gets put on various places and never removed.
