Multidisciplinary Care for Chronic Diseases?

I'm just sort of intrigued because, I mean, obviously it makes huge intuitive sense that thought. But what is the evidence? Is there an evidence base that we can draw on? And yet with due respect to mental health services who deliver through that model, you know, we don't. A lot of it's subject to long-standing turf wars in the Australian medical system and a lot of regulation at state level. What's been difficult is landing a system that's just not well set up to do it. There have been more pilots in this area than Qantas has.
