Some patients can come by ALS transport, but it's a dicey situation. I really feel that retrieving them there is the optimal way to do it. They're so much safer once they're on pump. Some of these patients need a significant resuscitation. There's that period of instability as you go on pump that can happen no matter how slowly you go on with your flows and support them.
