Taking Notes About Student Learning in a Classroom?

A lot of the training I give coaches really focuses on this. How do you take notes about a lesson such that it helps you and a teacher to think about what might be the next important thing? "I would talk right down things about learning and wherever it's happening and how it could happen better"
