How to Treat Syphilis During the Second Half of Pregnancy

When syphilis is diagnosed during the second half of pregnancy, management should include a detailed fetal level 2 ultrasound to look for congenital syphilis. Sonographic signs of fetal or placental syphilis include fetal hepatomegaly, ascites, hydrops, fetal anemia, or a thickened placenta. All of these indicate a greater risk for fetal treatment failure. The optimal interval for receiving benzanthine penicillin for those that are syphilis of unknown duration or late late and syphilis is one week apart.
