Most people believed in real presence but transubstantiation is the mechanism for real presence. There was a massive wall of division between clergy and laity that the Reformation reduced. Medieval western Christians, let's say late medieval western Christians were being starved of the Eucharist. It was given only in one kind - bread not wine typically to the laity.
