The most common cause of secondary adrenal insufficiency is exogenous gluco corticoid use. Tuberculosis would come to mind in a individual who might have some immuna compromise state. Any bilateral adrinal trama or hemorrhage can include histo plasmosis and crypto cacus. Some drugs that used in pretty high doses can inhibit a drenal corticle synthesis.
