Rheumatic fever is essentially what we call a disease of poverty or disadvantage. And it's likely a combination of things, in particular household crowding and limited access to healthcare. The rates are high in northern Australia, but despite that, there are occasional cases that just pop up in southern hospitals - even though they're not socioeconomically deprived.
