An independent investigator is the investigator of last resort. So i get to see all f the most extreme stuff, and i have to remember that, but commonly. Just about everybody who has worked on those sort of investigations concludes that the regulatorury landscape an their health service is incredibly complicated. Its really complicated to try and work out what to do for the best o cas. The trust is responsible to its lordo r, non exactanx, for the quality of the services that it provides. And the initial responsibility, if there are problems of the service, lies ft fairly and squarely with the trustnd.
