The diagnosis of sarcoidosis is arbitrarily made when the statistical likelihood of alternative diagnosis becomes too small to warrant further investigation. So I prefer a biopsy early, but I agree with you. I think sometimes patients don't want to do that and are looking for you to reassure them that the odds are with them.
