Long term capital management was set up by a couple of nobele prize winning economists. The fund collapsed through a combination of the 19 97 asian financial crisis, the 19 98 russian currency crisis and some bad planning. It's merely that before the events happened, their models said that such events were spectacularly rare. They were wrong.
