Two-thirds of the patients who underwent brain biopsy in our cohort with a primary indication to rule out small vessel CNS vasculitis were ultimately diagnosed with other ideologies that would not have required brain biopsy. The most common alternative eology was MoGAD followed by CNS-restricted HLH. Based on these findings, I will not recommend a brain biopsy for a suspected small vessel, primary angitis of the CNS, or honestly more broadly, neuroinflammatory disorders until I have the anti-mod, antibody results in hand.
