Disease can be transmitted during the kind of close contact that happens during sex. It's more likely just because of the close contact that goes along with sexual activity, not necessarily because of the biological act. Most of the spread happens from contact with the sores. The open sores are have a pretty high number of viruses in them, and so they're pretty infectious.
