Diuretic Resistance in Heart Failure Patients

The reason we use thiazides preferentially is this hypertrophy of the distal convoluted tubule where you get increased sodium reabsorption. What we do is we have a low threshold to add something like acetazoleamide. There is the AD4 trial. We'll use SGLD2 inhibitors acutely at certain times. Just for that extra boost of naturisis, those were shown to potentiate the naturitic effect of loop diuretics by about 20%.
