The Importance of Community Connections in Health Care
There are a lot of community connections to be had out there. Sometimes it's just identifying where those community resources are, making those connections. We can't truly care for patients holistically if we don't understand some of these social drivers of health. If you don't have utilities and your refrigerator is not working yet you're given a prescription for insulin that has to be refrigerated, that impacts how you can actually manage diabetes.
