Fouco breaks down history into long periods or epochs, that he calls epistemes. Instead of looking at individuals, they are just looking at movements and trins that have happened throughout history. When the fundamental language we use to describe the illness changes, so too does the way we see the illness and the patient and the task of the doctor. There's even some metaphysical claims in there.
