The Boeing 757 Accident
The crew was not trained to recognize, analyze and take proper actions in abnormal situations. The structure of the aircraft remained intact until impact at the water. No evidence of any pre-impact fire explosion or hostile activity. They'd been there for a month, they wanted to leave. I think they entered a situation they were unprepared for and did not deal with it appropriately.
