Type 2 Brugata Syndrome
The diagnosis of Brugata syndrome requires a Type 1 pattern or Type 2 that converts to Type 1 with sodium-channel blockade. Lead misplacement is much more common than Brugata Syndrome and can easily be identified by looking at the P-wave, which is normally by Phasic in V1 and fully upright in V2. The next step involves drawing a triangle whose peak is the R prime and whose base is 5mm below. If the triangle base is wide at greater than 4mm, this identifies Type 2 Brugata. And if the triangle Base is narrow, it is a normal variant.
