In both cases, there were automated systems that required humans to monitor the technology like that was the set up. Researchers have found that we're really bad at watching a machine and making sure the machine doesn't screw up. What does work is when the machine watches us and helps us not screw up. The autonomiust technology could save far more lives, i think, than the pipe dream that we're chasing of correcting for all human error.
