What Is Concurrent Documentation?

Concurrent documentation is a practice of completing or progress notes, or at least part of them, while we are in session with our clients. Many large agencies in California have moved toward a requirement or a recommendation for concurrent documentation for their clinicians. But again, if you are a treatment provider who uses a frequent intervention, that may be one of those cases where it's permissible.
