Physical exam findings may range from signs associated with a localized syndrome to hepatomegaly. If your patient's HIV is well controlled on anti-retroviral therapy, it may not be helpful to order them at admission. Despite the lengthy incubation period, culture remains the gold standard for definitive diagnosis of histoplasmosis.
