There was not clear channels of communication and protocol established to prevent something like that from happening. Air France 447 is a weird one because it basically disappeared over the Atlantic Ocean. That's just bizarre. Things just don't drop out of the sky like that. In fact, the middle of the flight is the safest part of the flight.
