I think when patients are in the hospital, it's probably just a little bit more practical and doesn't require repeated assessments. It does allow me to talk about one of my big nephrology pet peeves, which is multiples of maintenance. Is either a patient is uvelemic or they're not. To your point of bolus into uvelemia, it's okay to miss a couple hours of maintenance fluids if it means getting out of bed. The other thing that I often encourage my residents to think about is whether a patient truly needs IV fluids.
