How Do We Make Sure That This Never Happens Again?

Blame is always going to be there, because there's this sense of a slight or an injustice. You can't avoid feeling bad if it's a public incident more than when it's a private one. There's also something i like to call the shallow blamelessness where you decide that we don't name anybody but the fault is still with someone fucking up somewhere. Without naming names, all you got was anonymity. And so, you know, anonymity is like step one to oid having issues of people being mad of something like that. So you have to value them as good opportunities to better introspect the way you work and how things are happening in your organization.
