The History of Non-Adherence to Vasodilators
The patient had a longstanding history of pulmonary arterial hypertension since childhood. She'd only recently been started on the vasodilators that you just described in your introduction. We spent a lot of time very acutely trying to really clarify what medication she had been taking, what the doses were, and if she was taking them properly. Many patients can have RV dysfunction without overt signs of organ hyper-profusion.
