How to Speak Up About Errors
i was interested in, in a general sense, i was interested in the learning organization. You know, how how do we learn? And of course, one of the things that the learning organization needs is the capacity to learn from mistakes. So i was very excited to be invited to join a m of medical researchers who were studying medication errors in n in two local hospitals. I'm going to get in there and i'm going to understand it better. That's what i was looking for, but i stumbled into, by accident, quite profound differences in reporting climate.
