The Maintenance Engineer Didn't Turn His Attention to the Washer
The engineer had lunch during the span of time it took to complete the engineering order. It's possible he didn't know there was a washer on there, and the washer just kind of fell off. He did not report to the supervisor that the front bolt of the slat track needed to be etached during the maintenance work. And he did not remember whether or not there were any loose parts or clearance after finishing the job. Because the nut was slightly smaller than the hole it was installed on, the entire downstop just fell out through the hole it's attached in.
