You can only do that analysis if you have a gold standard. So what's the gold standard in that situation? The patient gets the fluid and they see if the patient responds the next day. You can then get an answer that may be anywhere from 70 to 90% sensitive and specific. To answer to find out what would happen if I just gave the patient a bag of saline.
