Awareness Under Anesthesia

One of the major fears about anaesthesia is that, could some one be awake, but anaesthesiologess not perceive it? We currently realize that that happens one to two in ten to 20 thousand cases of anaesthesia. To my thinking, it's something which should no longer ever occur if patients are properly monitored during surgery.
