When biologics first came out, the recommendations were still that this is a pyramid approach. These drugs were really for the sickest of the sick, the people who had failed other therapy. Now, unfortunately, we see them being at being used much more first line. If you had told me 25 years ago I would be treating complicated autoimmune diseases like Crohn's and Ulster colitis using diet and lifestyle things, basic things, I would have laughed at you.
