To be idealistic to get worn down by something means to become weaker because of difficult experiences over time. A quack is basically a fake doctor a fraudulent doctor who would probably just do things for money rather than genuinely trying to help people so he becomes a bit more cynical and he becomes a quack okay q u a c k exactly the same word as the noise that a duck makesQuacks are also known as "fake doctors" or "fraudulent doctors" in terms of their work.
