Cystocytes in Streptococcus Pneumonia-Associated HUS

A peripheral smear will show the presence of cystocytes, which consists of fragmented, deformed, irregular red blood cells. Smir may also show giant platelets, which are due to rapid platelet turnover from peripheral destruction. In pneumococcal HUS, where there is antigen-antibody interaction on the red blood cell surface, the direct cooms test may be positive in 90% of cases.
