A really, really good cohort study might be more valuable than a somewhat poorly done r c t. And then they just lop off the top of the pyramid. Ah, so systematic reviews and metaanalyses are no longer on the pyramid itself. It's kind of the the handle of a magnifying glass that you can peer through to look at the other types of evidence.
