Is There a Requirement for Documentation?

Am, one of the things that really brings me pause on this issue is as i think back to when i first sought medical intervention for geneisphoria and wanted to transition. I was never informed of any a thing i would have to do to maybe get my health to a certain place before undergoing such a radical, you know, procedure. Um, there was never any discussion of the actual transition plan. There was never any requirement for documentation from a therapist. Back then, things were more conservative with how we treatedgender transition, and now it's even looser.
