SMA patients with neuromuscular disease, they have a lower FRC in that predisposism, the adelectasis. They also have the tendency towards these inefficient paradoxical chest wall movements which really makes a ventilation ineffective. The reason peep works from an oxygenation standpoint is because it increases the total amount of pressure. But remember, if you're trying to oxygenate with peep, it's just a physiologic trick. It's not solving the problem.
