What's Going on With the Kidneys and Heart?

The patient has a history of hypertension. His blood pressure was 202 over 102 at the time, which he said was near his recent baseline off his blood pressure medications. He denies metallic taste in the mouth, pyridus, restless leg, hiccups, confusion or nausea and vomiting. The one piece of information here that is interesting and striking is this recent upper respiratory infection.
