The way that we start Naltrexone at our hospital is that we start 25 milligrams on day one, but I do have to say that this is not evidence based. And you can also just start at 50 milligrams and that is completely fine. The reason that I do a lower dose initially is that the GI side effects, so like abdominal pain, nausea, diarrhea, which I often go away over time or decreased if you start with lower doses. Some clinicians, if they're really worried about somebody not continuing NaltrexONE because of the side effects, may actually start 12.5 milligrams daily for seven days and then slowly titrate up.
