Flight Crews Failed to Recognize Whole Lines at Oscar for Intersections
The NTSB found that the whole lines for Oscar for were parallel to runways instead of perpendicular to taxiways. There was also an absence of runway edge lights on runway three center at the Oscar for runway intersection. Flight crews expect whole lines to be at right angles to taxiway center lines. And in this accident, the DC nine crew may have seen the yellow markings, but they could have failed to recognize them as whole lines because of the angle relative to their taxi path.
