The KLM Captain Ah Didn't Take Off Without Clearance
The spanish investigations commission determined that the k l m captain ah to take off without clearance and his desire to leave as soon as possible. The sudden fog that limited the visibility of the two flight crews and tower interference on the radio resulted in k l m not hearing the towers transmission to not take off. And also, there's that ambiguous phrase, i dont know if you heard it, when k l m read back the instructions, at the end of their instructions, they said, we're at take off, and the towers responsive. Also contributed were the airport was being forced to accommodate more traffic than it was designed to handle.
