E e a had design review shepherds, or something a that we sort of set up. It wasn't designed to be a gate keeping process, but rather a like, if you're designing a thing and you want some eyes on it, here's how you get some help. We ran code reviews as well; i can't think we used review board in my time. But making sure that, again, we're not just pick a tool and deploy it is part of the problem.
