The Importance of Preload Dependence in Pulmonary Hypertension

There's clearly a big difference between someone who gets admitted for volume overload and someone like her who's in shock. I think the term, and I hope I don't say anything, Dr. Gomberg disagrees with, but I'd be still very interested is when people say that pulmonary arterial hypertension patients are preload dependent. The reality is because of particular independence, again, with the septum being pushed over into the LV, the LV is underfilled. When we reduce preload, it actually gives the LV space to be filled more adequately. And as long as you're measuring your electrolytes continuously, it works a lot faster.
