There's two main ways that a muscle would hypertrophy. In either case, the muscle cell got larger. A contractile hypertrophy happens as a result of increasing proteins on those myosin and actin. And so probably the biggest explanation for why muscle increases in its diameter is exactly that you've put more proteins in the contract units to maintain optimal spacing.
