There can just be progression of their underlying disease either because it has not yet been diagnosed and as you probably are aware, the signs and symptoms of pulmonary hypertension are not this way specific. And so especially in younger patients and younger women who develop pulmonary hypertension, they may go undiagnosed for quite a while until they present in our decompensated states. Unfortunately, we not infrequently see iatrogenic causes of decompensation. It can be either because providers are unaware of let's say the risks associated with anesthesia. If there's not an awareness ahead of time about the risk, we're clearly aware that either a patient's not adherent or accidental withdrawal of their pulmonary vasodil
