resistant hypertension is when blood pressure is not controlled below 140 over 90, despite treatment with optimal doses of at least three drugs. 25% of these patients actually have what's called white coat hypertension - their blood pressure is elevated in the office but normal away from the office. You need to do ambulatory blood pressure monitoring ideally or home blood pressure monitoring to exclude that phenomenon. The final issue is the issue of adherence or compliance to therapy. We were horrified when we started doing urine testing to find about 40% of patients on at least 3 drugs were not taking at least one of them. And if you can sort that out, we know that 90% of patients will be controlled on three drugs.
