An ectopic pregnancy can often present in an atypical manner. This patient's quantitative serum beta HCG was actually over 10,000 and no evidence of an IUP on ultrasound. The provider might have anchored on the lack of vaginal bleeding which steered them away from a pregnancy-related cause of pain.
