The regions responsible for identifying fear stimuli and generating fear responses downstream can be hyperactive, underactive or have structural abnormalities in anxious patients. The medial prefrontal cortex provides top down regulatory control of fear responding by receiving input from the thalamus and projecting to the amygdala. If the dorsal and anterior cingulate cortex is damaged, patients will struggle to associate certain actions with emotions such as fear and sadness. This kind of work is incredibly valuable in finding pharmacological targets for potential medications.
