Non-invasive ventilation has found a clear role in the world of ARDS. How far you might push CPAP before biting the bullet and tubing them remains to be defined. It seems that if they're feeling extubation due to Eilert-Puln-Miodema or bronchospastic COPD type process then it's probably worth a go.
