Scott Simon: I'll start with mistakes I made running airlines. The first one that comes to mind is when we had a customer at Spirit who was told by his doctor he couldn't fly. We refused him his refund because he had bought a non-refundable ticket. Initially, I supported my team's approach on that. But then I realized that wasn't the right thing to do and have been paying for it ever since.
